5. Photocopy of Passport – ALL pages including cover (from first to last page and all page numbers should be clearly seen)
6. Scanned copy (in colour) of academic results
7. Scanned copy(in colour) of page of passport with personal particulars and photograph
8. Affidavit: If names in passport are different from names on academic certificates (to prove that the applicant is the same) For example in the passport the names stated are John Paul Smith and on the academic certificate the names stated are John P. Smith, then an affidavit is required.
9. Medical report(use approved format ATTACTHED
10. Yellow fever certificate. Inoculation should be taken atleast 2 weeks before travelling.(for students from Africa and South America)
11. A bank draft of USD300.00 for the application fee(non- refundable) payable to:
